If you move house or change any other details (e.g. name, telephone number), please let us know as soon as possible. It is essential that your GP has the correct up to date information in the event that he/she needs to contact you.
If any of your personal details need to be amended, we request that you attend the practice with two of the following documents to verify your new details:
- Driving Licence
- Passport
- Utiliy Bill
- Bank/Building Society Statement(s)
- Mobile Phone Bill
- Letter from Benefits Agency
- Papers from the Home Office
